Background Request Form

If you are an Investigator requesting a background check on a current or former Georgetown University student please follow the steps enumerated below:

1. Download and complete the downloadable Background Request Form.
2. Submit the completed downloaded Background Request Form and authorized release form via email to Detective Janelle Clark or via fax at 202-687-8160, attention Investigations Unit.
